Benazepril and hydrochlorothiazide and Enalaprilat interaction

Benazepril and hydrochlorothiazide and Enalaprilat: both prescribing labels flag this combination with the strongest language, such as a contraindication, a boxed warning, or an instruction to avoid it.

Major

Do not combine without a prescriber's guidance. The label language below is the strongest FDA uses.

What the FDA labels say

From the Benazepril and hydrochlorothiazide (Lotensin HCT) label · effective 2026-02-24

Major

Lotensin HCT is also contraindicated in patients who are hypersensitive to benazepril, to any other ACE inhibitor, to hydrochlorothiazide, or to other sulfonamide-derived drugs.

Contraindicationsby drug classView label on DailyMed
Moderate

Like other ACE inhibitors, benazepril has been only rarely associated with hypotension in uncomplicated hypertensive patients.

Warningsby drug classView label on DailyMed
Moderate

The thiazide component of Lotensin HCT may potentiate the action of other antihypertensive drugs, especially ganglionic or peripheral adrenergic-blocking drugs.

Warningsby drug classView label on DailyMed
Moderate

Other ACE inhibitors have had less than additive effects with beta-adrenergic blockers, presumably because drugs of both classes lower blood pressure by inhibiting parts of the renin-angiotensin system.

Drug interactionsby drug classView label on DailyMed
Minor

Lotensin HCT should be used cautiously in patients receiving concomitant therapy with other antihypertensives.

Warningsby drug classView label on DailyMed

From the Enalaprilat label · effective 2024-01-04

Major

…sometimes associated with oliguria and/or progressive azotemia, and rarely with acute renal failure and/or death, include those with the following conditions or characteristics: heart failure, hyponatremia, high dose diuretic therapy, recent intensive diuresis or increase in diuretic dose, renal dialysis, or severe volume and/or salt depletion of any etiology.

Warningsby drug classView label on DailyMed
Moderate

WARNINGS Hypotension Excessive hypotension is rare in uncomplicated hypertensive patients but is a possible consequence of the use of enalaprilat especially in severely salt/volume depleted persons such as those treated vigorously with diuretics or patients on dialysis.

Warningsby drug classView label on DailyMed
Moderate

It may be advisable to eliminate the diuretic, reduce the diuretic dose or increase salt intake cautiously before initiating therapy with Enalaprilat Injection in patients at risk for excessive hypotension who are able to tolerate such adjustments (see PRECAUTIONS, Drug Interactions ; ADVERSE REACTIONS, and DOSAGE AND ADMINISTRATION ).

Warningsby drug classView label on DailyMed
Moderate

Because of the potential for an excessive fall in blood pressure especially in these patients, therapy should be followed closely whenever the dose of enalaprilat is adjusted and/or diuretic is increased.

Warningsby drug classView label on DailyMed
Moderate

Drug Interactions Hypotension - Patients on diuretic therapy Patients on diuretics and especially those in whom diuretic therapy was recently instituted, may occasionally experience an excessive reduction of blood pressure after initiation of therapy with enalaprilat.
