Benazepril and hydrochlorothiazide and Fosinopril interaction

Benazepril and hydrochlorothiazide and Fosinopril: both prescribing labels flag this combination with the strongest language, such as a contraindication, a boxed warning, or an instruction to avoid it.

Major

Do not combine without a prescriber's guidance. The label language below is the strongest FDA uses.

What the FDA labels say

From the Fosinopril (Fosinopril Sodium) label · effective 2024-05-16

Major

CONTRAINDICATIONS Fosinopril sodium tablets are contraindicated in patients who are hypersensitive to this product or to any other angiotensin-converting enzyme inhibitor (e.g., a patient who has experienced angioedema with any other ACE inhibitor therapy).

Contraindicationsby drug classView label on DailyMed
Moderate

In separate single or multiple dose pharmacokinetic interaction studies with chlorthalidone, nifedipine, propranolol, hydrochlorothiazide, cimetidine, metoclopramide, propantheline, digoxin, and warfarin, the bioavailability of fosinoprilat was not altered by coadministration of fosinopril with any one of these drugs.

Drug interactionsnames Benazepril and hydrochlorothiazideView label on DailyMed
Moderate

Like other ACE inhibitors, fosinopril has been only rarely associated with hypotension in uncomplicated hypertensive patients.

Warningsby drug classView label on DailyMed
Moderate

Symptomatic hypotension is most likely to occur in patients who have been volume- and/or salt-depleted as a result of prolonged diuretic therapy, dietary salt restriction, dialysis, diarrhea, or vomiting.

Warningsby drug classView label on DailyMed
Moderate

In such patients, fosinopril sodium therapy should be started under close medical supervision; they should be followed closely for the first 2 weeks of treatment and whenever the dose of fosinopril or diuretic is increased.

From the Benazepril and hydrochlorothiazide (Lotensin HCT) label · effective 2026-02-24

Major

Lotensin HCT is also contraindicated in patients who are hypersensitive to benazepril, to any other ACE inhibitor, to hydrochlorothiazide, or to other sulfonamide-derived drugs.

Contraindicationsby drug classView label on DailyMed
Moderate

Like other ACE inhibitors, benazepril has been only rarely associated with hypotension in uncomplicated hypertensive patients.

Warningsby drug classView label on DailyMed
Moderate

The thiazide component of Lotensin HCT may potentiate the action of other antihypertensive drugs, especially ganglionic or peripheral adrenergic-blocking drugs.

Warningsby drug classView label on DailyMed
Moderate

Other ACE inhibitors have had less than additive effects with beta-adrenergic blockers, presumably because drugs of both classes lower blood pressure by inhibiting parts of the renin-angiotensin system.

Drug interactionsby drug classView label on DailyMed
Minor

Lotensin HCT should be used cautiously in patients receiving concomitant therapy with other antihypertensives.
