Overestimating the butorphanol tartrate dosage when converting patients from another opioid product can result in a fatal overdose with the first dose.

Additionally, avoid the use of mixed agonist/antagonist (e.g., pentazocine, nalbuphine, and butorphanol) analgesics in patients who are receiving full opioid agonist analgesic, including Oxycodone Hydrochloride Oral Solution.
