Fosinopril and hydrochlorothiazide and Quinapril interaction

Fosinopril and hydrochlorothiazide and Quinapril: both prescribing labels flag this combination with the strongest language, such as a contraindication, a boxed warning, or an instruction to avoid it.

What the FDA labels say

From the Fosinopril and hydrochlorothiazide (Fosinopril Sodium and Hydrochlorothiazide) label · effective 2024-04-28

Major

Fosinopril sodium and hydrochlorothiazide is also contraindicated in patients who are hypersensitive to fosinopril, to any other ACE inhibitor, to hydrochlorothiazide, or other sulfonamide-derived drugs, or any other ingredient or component in the formulation.

Contraindicationsby drug classView label on DailyMed
Moderate

Like other ACE inhibitors, fosinopril has been only rarely associated with hypotension in uncomplicated hypertensive patients.

Warningsby drug classView label on DailyMed
Moderate

The thiazide component of fosinopril sodium and hydrochlorothiazide may potentiate the action of other antihypertensive drugs, especially ganglionic or peripheral adrenergic-blocking drugs.

Warningsby drug classView label on DailyMed
Moderate

Other ACE inhibitors have had less than additive effects with beta-adrenergic blockers , presumably because drugs of both classes lower blood pressure by inhibiting parts of the renin-angiotensin system.

Drug interactionsby drug classView label on DailyMed
Minor

Fosinopril sodium and hydrochlorothiazide tablets should be used cautiously in patients receiving concomitant therapy with other antihypertensives.

From the Quinapril label · effective 2024-05-15

Major

…sometimes associated with oliguria and/or progressive azotemia, and rarely with acute renal failure and/or death, include patients with the following conditions or characteristics: heart failure, hyponatremia, high dose diuretic therapy, recent intensive diuresis or increase in diuretic dose, renal dialysis, or severe volume and/or salt depletion of any etiology.

Warningsby drug classView label on DailyMed
Moderate

No pharmacokinetic interaction was observed when single doses of quinapril hydrochloride and hydrochlorothiazide were administered concomitantly.

Drug interactionsnames Fosinopril and hydrochlorothiazideView label on DailyMed
Moderate

It may be advisable to eliminate the diuretic (except in patients with heart failure), reduce the diuretic dose or cautiously increase salt intake (except in patients with heart failure) before initiating therapy with quinapril hydrochloride in patients at risk for excessive hypotension who are able to tolerate such adjustments.

Warningsby drug classView label on DailyMed
Moderate

Such patients should be followed closely for the first two weeks of treatment and whenever the dose of quinapril hydrochloride and/or diuretic is increased.

Warningsby drug classView label on DailyMed
Moderate

If symptomatic hypotension develops, a dose reduction or discontinuation of quinapril hydrochloride or concomitant diuretic may be necessary.
